Special Terms and Conditions <br />i. The student has reached the agreed-upon goal for.the.number of weekly hours worked' <br />unless the Student and the DVR CounTJl'igt"" that the job represents substantial and <br />suitable emPloYment' <br />ii. The student has reached a maximum level of on-the-job employment and meets <br />emPloYer exPectations' <br />iii. The supports available are adequate for the student to maintain stable employment and <br />t""ttnd "mployur;s <br />expected level of work productivity' <br />iv. The student has long-term employment supports available or in process and is able to <br />transition tu these sJrvices to maintain job stability' <br />(c)onceachieved,aStabilizationReportandsupports^lnthgjobtheStudentmayneed,must <br />becompleteotortnestudentasoutlinedinthesDoPandmustinclude: <br />i. Detail and summary of how the placement hours were met (or if the job represents <br />suitable employment); and <br />ii. Detail, descriptions, and summary of how the DVR Counselor, CRP, and the employer <br />;g;;; tnat ftre emptoyers expeciation have been met; and <br />iii. Detail and summary of how long-term supports have been set up and will occur; and <br />iv. The strengths the Student brings to the job' and <br />V.TheStudent,sfadingplan,includinghowtheywillremainstableontheiob;and <br />vi. Additionaltraining, services, and supports the Student may need' <br />(4)StabilizationneedstobeachievedbySeptember30,2o2T.However'exceptionsmay <br />be made on a case-by-case nasis tliough tne_ena of the calendar year (December 31' <br />2027) withapprovat from the DVR i;;;ii"; Pl.ogr"r Manager or their designated <br />representative. Youth Extended Services' <br />(a) DVR shall provide Youth Extended services needed by the student until DDCS funds are <br />available, in accordance with WAC 388-891A-1060' <br />i. should youth Extended services reach six (6) months, the contractor and DVR shall <br />collaborate to determine next steps *ni"f'., ni"l include additional Youth Extended <br />Services or other options as needed' <br />(b) Youth Extended services funded by DVR shall be outlined on the DVR Youth Extended <br />Services SDOP' <br />i.
