■ Support clients in finding housing, negotiating leases, and coordinating move -ins. <br />■ Offer ongoing support to maintain stable housing, including landlord mediation and <br />conflict resolution. <br />Follow Up and Evaluation <br />■ Track and report on key metrics such as the percentage of clients who complete the <br />goals on their stability plan, the average number of goals achieved by program exit, <br />housing retention and the percentage who remain housed after program exit, the <br />number of directly provided services, and the percentage of referrals acted upon. <br />■ Review client outcomes every six months. <br />■ Gather feedback regularly and adjust the program as needed to meet clients' evolving <br />needs. <br />■ Regularly engage with community partners to ensure the successful completion of <br />closed- loop referrals. <br />Community and Stakeholder Engagement <br />■ Collaborate with local organizations to work collectively to improve the community's <br />mental, physical, and emotional health. <br />■ Engage with the local community to raise awareness of the program and connect <br />participants to additional resources. <br />■ Participating in local or regional advocacy efforts to improve the lack of awareness <br />and educational material available around behavioral health and housing. <br />Kittitas County Professional Services Agreement <br />Page 16 of 22 <br />
