EXHIBIT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following <br />activities: <br />Project Planning and Coordination <br />• Negotiate and formulate contract with behavioral health partner <br />• Formalize and strengthen relationships with organizations that are involved in <br />providing services to program participants. <br />Whole Person Care Model Implementation <br />• Care Navigation and Case Management <br />o Conduct comprehensive needs assessments to identify barriers related to <br />behavioral health, substance use, housing, income, employment, food <br />access, transportation, social connection, and interpersonal safety. <br />o Make closed -loop referrals to and coordinate with community partners to <br />ensure clients receive the necessary support for services external to the <br />program. <br />o Develop personalized stability plans with clients, focusing on long-term <br />stability goals (housing, employment, health) <br />o Regularly review and adjust clients' goals, providing tailored support and <br />ensuring accountability. <br />o Offer workshops or one-on-one support to develop life skills for budgeting, <br />managing household responsibilities, and navigating social services. <br />o Involve certified peer support specialists who have lived experience to offer <br />emotional support and guidance. <br />Bridge Behavioral Health Support <br />• Conduct comprehensive mental health assessments upon program entry. <br />• Offer regular one-on-one counseling and behavioral health services with the <br />contracted provider. <br />■ Navigate and connect to long-term behavioral health resources. <br />Housing Access and Support <br />• Rapid Housing Placement- Identify and secure permanent housing for participants <br />without preconditions. <br />Kittitas County Professional Services Agreement <br />Page 15 of 22 <br />
