The lntake Specialist greets the guest and explains the intake process, which is required for first- <br />time guests in the current season. The Specialist highlights the shelter's low-barrier model and <br />briefly goes over the services provided, including the importance of maintaining records for safety <br />and resource allocation. <br />Apricot lntake Process <br />The lntake Specialist begins by collecting basic information about the guest to meet the Homeless <br />Management lnformation System (HMIS) reporting requirements. This will include: <br />Household Profile: includes gathering names, relationships, and other pertinent details for all <br />individuals that are a part of a household group. There may be one person households. <br />Participant Profile: demographic information about the specific guest. <br />lncome Form: guests provide details about their financial situation. This helps in understanding <br />the guest's potential needs and eligibility for further assistance, though it does not affect their <br />ability to access the shelter. <br />HMIS Prosram Form: The lntake Specialist completes an HMIS Program Form to ensure that the <br />guest's details are entered into the shared database, tracking the services provided and ensuring <br />coordinated care with other service providers. <br />Program Enrollment: The guest is officially enrolled in the CWS program by completing the <br />Program Enrollment form. <br />Guest Guidelines: The lntake Specialist carefully reviews the CWS Guest Guidelines with the guest, <br />explaining the expectations for behavior, safety protocols, and general shelter rules. The guest is <br />asked to acknowledge their understanding of the guidelines by signing the agreement in Apricot. <br />For detailed steps and screenshots, please refer to the tango guide. <br />Finalizing lntake <br />Once all forms are completed and the guidelines are signed, the guest is welcomed to the shelter. <br />The lntake Specialist provides information about the assigned sleeping area, meals, and any <br />immediate resources they might need. <br />Guest Rights and Responsibilities <br />At the CWS, all clients are treated with dignity, respect, and compassion, and have the right to a <br />safe, non-judgmental environment where their privacy and confidentiality are protected.
