Consultation/ <br />Training <br />System Improvements <br />Wise provides a range of consultation efforts Hourly rates <br />for government partners. Examples include: <br />• Employment policy development <br />• Employment outcome analysis <br />Community TA and Training plans <br />County PATH Plan <br />Wise Training and TA staff will facilitate a <br />graphically recorded path plan meeting that <br />focuses on services provided in your <br />community. Ongoing support and follow-up <br />support will be provided by the 2 staff <br />facilitators. One additional TA staff with <br />identified expertise to support the <br />implementation of the plan will be available. <br />Positive Behavior <br />Provides Employment Professionals with the <br />Support <br />unique opportunity to learn how to be <br />Specialization <br />Positive Behavior Support Specialists for <br />Cohort <br />i their agencies and others. <br />Providers who complete this training are <br />prepared to consult, train, and support other <br />community -based providers with PBS <br />principles. <br />Investment Projects: Creating stories approximately 10 minutes in <br />Equity and Diversity length, promoting employment and inclusion <br />Video Series statewide. Priority will be given to historically <br />marginalized or under -served communities <br />and community members. <br />Organizations wishing to participate will <br />receive training and support to assist them in <br />Professional Services Agreement <br />Page 23 <br />$11,000 for submission of <br />completed PATH Plan and <br />$11,000 at the completion of th <br />6-month period. <br />$5,500 per seat <br />$20,000 per agency registratior <br />• $10,000 to Wise and <br />$10,000 to agency <br />
