3. County Priorities: lndicate which of the following priorities are being addressed by the <br />proposed project and how you intend to address them. (Select all that apply and give <br />a brief L00 word description for each selected.) <br />Fluickly identify and engage people experiencing homelessness. <br />Outreach and identification is accomplished through field visits to all areas and <br />businesses where homeless individuals or families are likely to spend time. We track all <br />outreach efforts on a weekly basis to assure anyone experiencing homelessness is <br />identified and given continued opportunities to receive assistance. We work closely with <br />the City Homelessness Task Force to come alongside individuals identified as <br />homeless with high vulnerability and currently un-engaged needing services. lntakes <br />include a VI-SPDAT designed for each demographic we serye in order to determine <br />each clients level of vulnerability and to develop a stability plan best suited to the needs <br />of each client. <br />r'Prioritize housing for people with the greatest needs <br />Affordable housing prioritizes attainable housing for low- and very-low income <br />households. ln the case of PSH units, priority is given to chronically homeless <br />households that include a person with a disability. Prioritization will be based on the <br />length of time homeless and the severity of the household's needs. <br />Operate an effective and efficient homeless crisis response system that <br />swiftly moves people into stable permanent housing. <br />The Kittitas County Continuum of Care Coordinated Entry System for homeless <br />individuals has been created to better maximize resources available in the community <br />and to quickly re-house homeless individuals or prevent eviction. Using a web-based <br />entry system that can be accessed from any computer or phone, the system is <br />positioned to deliver information to the entire housing provider network immediately in <br />one step, avoiding the need to go "door to door" for assistance. Agencies then <br />communicate among themselves to determine the best choice for the applicant and to <br />prioritize services.
