<br />KCHD ORDINANCE 1999- 30 SOLID WASTE REGULATIONS <br />(i) The company name, contact name, address and telephone number of the generator, <br />the generator’s consultant, the generator’s contractor, and the analytical laboratory <br />providing data to support the application. <br />(ii) The site name, street address, city, county and state where the waste proposed for <br />disposal originated. If the site is a remedial action identified by EPA or Ecology, <br />the name and telephone number of the site manager representing the respective <br />agency shall be included. <br />(iii) A description of how the waste was generated. <br />(iv) A description of the physical and chemical nature of the waste proposed for disposal <br />including a description of the potential waste contaminants. <br />(v) Whether the application is for a one-time or on-going disposal authorization. <br />Applications for on-going disposal must identify the procedures used to ensure that <br />the application being submitted will remain representative of the waste proposed for <br />disposal. <br />(vi) The quantity of waste proposed for disposal in cubic yards and tons and the manner <br />in which the waste is stored on-site. Applications for on-going disposal must <br />identify the quantity of waste that will be disposed of per month, or other frequency <br />if monthly quantification cannot be achieved due to the manner in which the waste is <br />generated. <br />(vii) A checklist identifying that each requirement of the waste designation procedures in <br />WAC 173-303-070(3) was followed by the generator. <br />(viii) A description of the sampling frequency and sampling method. The sampling <br />frequency recommended in Table I of Ecology document 91-30, “Guidance for <br />Remediation of Petroleum Contaminated Soils” (April 1994 and as amended) shall <br />be used. If the number of samples collected differs from the number recommended <br />in Table I of this reference or required in the Health Department approved waste <br />screening program a narrative shall be included which provides justification that the <br />number of samples collected is representative of the entire waste stream proposed <br />for disposal. <br />(ix) A description of the analytical procedures selected to designate the waste proposed <br />for disposal including justification why certain parameters were chosen for analysis <br />and others excluded from analysis. Analyses shall be performed by a professional <br />laboratory accredited by the Washington State Department of Ecology waste water <br />accreditation program, or other equivalent accreditation program(s), for the <br />parameters tested. Equivalency of accreditation programs must be determined by <br />the facility as part of the approved waste screening program. Analytical methods <br />used must be those identified in the following Ecology and EPA guidance <br />documents and their respective amendments: <br />(aa) Ecology document 80-12, “Biological Testing Methods”. <br />(bb) Ecology document 91-30, “Guidance for Remediation of Petroleum <br />Contaminated Soils”.
