Spoke model connect a network of comnunity providers around a central hub, currently located <br />in Yakima, Washington, offering a MAT component to all patients seeking treaknent. Kittitas <br />County Jail participates in this program as a Spoke, providing treatrnent referrals to the state- <br />funded Hub entity. While participation in this program has created an avenue for access where <br />one did not previousty exist the Hub entity can only provide minimal services in the Jail on a <br />pafi-time basis, Cun-ently the program seryes approximately 12-19 inmates on a part-tirne basis, <br />but a full-time program is needed to adequately address the needs of the estinated 30% of <br />inmates (28+ inmates based on the average daily population) in need of keatment. A full-time <br />Nurse Care Manager $fCM) and peel support specialist on-site would allow for adequate <br />coverage of MAT treahnent, care coordination, and community referrals to support services. <br />Kittitas County Jail is located in Ellensburg, Washington. As of July 2}lg, Kittitas <br />Counfy has an estimated population af 47,935t .
