<br />12/20/2016 MINUTES 4 <br /> <br /> <br /> <br />RESOLUTION 2016-170 GRANT COUNTY PUBLIC HEALTH <br /> <br />Approve Resolution No. 2016-170, Authorizing an Amendment to an <br />Interagency Agreement between the Kittitas County Public Health <br />Department and Grant County, in the amount of $15,000.00 for the North <br />Central Washington Healthy Communities Obesity, Diabetes, Heart Disease <br />and Stroke Prevention Program from January 1, 2015 through September 29, <br />2018. <br /> <br />CONTRACT WASHINGTON STATE HEALTH CARE AUTHORITY PUBLIC HEALTH <br /> <br />Approve Amendment No. 1 to Contract Number K1407 between the Washington <br />State Health Care Authority and the Kittitas County Public Health <br />Department, for Medicaid related Outreach and Linkage Activities from <br />January 1, 2017 through December 31, 2018. <br /> <br />RESOLUTION 2016-171 COMMUNITY NETWORK LEASE PUBLIC HEALTH <br /> <br />Approve Resolution No. 2016-171, Authorizing a Lease Agreement between <br />Kittitas County and the Kittitas County Community Network, in the annual <br />amount of $1,746.00 for an office located in Room 122, Morris Sorenson <br />Building 507 N. Nanum Ellensburg, WA beginning December 21, 2016 through <br />November 30, 2018. <br /> <br />AGREEMENT COMPREHENSIVE HEALTHCARE SHERIFF <br /> <br />Approve an Agreement between Kittitas County Sheriff’s Office and <br />Comprehensive Healthcare to provide a qualified Practitioner who will <br />provide Mental Health related Medical Assessment and Treatment to <br />inmates at the Kittitas County Corrections Center, at a rate of $125.00 <br />per hour through December 31, 2017. <br /> <br />AGREEMENT CITY OF KITTITAS – VEHICLE SHERIFF <br /> <br />Approve a Revocable License Agreement between Kittitas County and the <br />City of Kittitas, for a 2007 Ford Crown Victoria Police Interceptor, <br />Unit #0702, VIN 2FAFP71W87X146763, and to be removed from service by the <br />Sheriff’s Department and a replacement vehicle issued. <br /> <br />RESOLUTION 2016-172 CONSOLIDATED FOOD MANAGEMENT, INC. SHERIFF <br /> <br />Approve Resolution No. 2016-172, Authorizing an Option of Renewal <br />between Kittitas County and Consolidated Food Management, Inc. for the <br />Kittitas County Corrections Center from January 1, 2017 through December <br />31, 2017. <br /> <br />
