(c) The number of outreach materials developed and distributed including <br />news publications. <br />(d) The number of pharmacies involved in information dissemination <br />efforts. <br />(e) The number of health care providers involved in information <br />dissemination efforts. <br />(f) Pounds of medicine collected monthly once a secure medicine take- <br />back box is in place. <br />(g) The number of home medication storage devices purchased and <br />distributed including a pre/post survey for those that received the <br />home storage devices to be approved by HCA/DBHR. <br />(h) Coalition coordinator time spent on the project in the "direct" and <br />"indirect" staff time related to the project. <br />5. Reporting Requirements. <br />a. Prevention Reporting Requirements <br />The Contractor shall report on all requirements as identified in the HCA Substance <br />Use Disorder Prevention and Mental Health Promotion Online Reporting System or <br />Minerva. HCA reserves the right to add reporting requirements based on <br />requirements of grants. <br />c. Prevention Activity Data Reports <br />The Contractor shall: <br />(1) Ensure that monthly prevention activities are reported in the HCA Minerva <br />in accordance with the requirements and timelines set forth. <br />(2) Ensure accurate and unduplicated reporting. <br />(3) Ensure proper training of staff and designated staff for back-up Minerva <br />data entry to meet report due dates. <br />(4) If special circumstances arise and Contractor is unable to enter the data <br />by the reporting deadline(s), the Contractor shall ensure any requests for <br />extensions to reporting deadlines are requested in writing and sent <br />directly to the PSM via email five (5) working days before the report due <br />date. <br />(5) The maximum extension request permitted is ten (10) working days. <br />(6) Monthly invoices submitted with active data entry extensions will be <br />denied and may be re-submitted by Contractor once data for the month(s) <br />in question is complete. <br />(7) Contractors with three (3) or more consecutive months of data entry <br />extensions or late reporting or four (4) or more program data entry <br />extensions or late reporting within a six (6) month period shall be required <br />to submit a Corrective Action Plan to HCA. <br />Washington State <br />Health Care Authority <br />(a) Extensions granted due to Minerva technical issues will be excluded <br />from this count. <br />Page 66 of90 HCA Contract No. K3924
