Exhibit D <br />Partnerships for Success (PFS) Secure Medicine Take -Back <br />Year 5 September 30, 2017 to September 29, 2018 <br />1. Definitions. The following definitions are added in this Exhibit: <br />a. "Rx" means prescription drugs and medications. <br />2. Purpose. <br />The purpose of the Year 5 Partnerships for Success Targeted Enhancement Project is to provide <br />support to the Community Prevention and Wellness Initiative (CPWI) community to implement a Secure <br />Medicine Take -back Project. The project aligns with the community needs assessment and will <br />increase local capacity to address prescription drug misuse and abuse by reducing social availability of <br />prescriptions in the community. <br />3. Statement of Work. <br />The Contractor shall provide the services and staff, and otherwise do all things necessary for or <br />incidental to the performance of work, as set forth below: <br />a. Enhance community capacity to practice safe disposal of medications by promoting permanent <br />secure drop box in the location where community readiness has been established. (Installation and <br />disposal must follow all DEA rules and all federal and state laws and regulations.) <br />(1) Collaborate with community partners to maintain and/or enhance policies and procedures <br />necessary to maintain a permanent secure medicine take -back drop box. <br />(2) Overtime wages for law enforcement officers and staff as outlined in strategic plan and outside <br />of normal duties and other real costs (including mileage reimbursement) associated with <br />transporting and properly disposing of collected medicines at EPA approved locations shall be <br />permitted. <br />b. Create, utilize and disseminate public education information materials to increase awareness of the <br />secure medicine take -back project, local treatment resources, naloxone information and medical <br />response (Good Samaritan law) cards. <br />(1) Collaborate with community partners to develop and adopt policies and procedures necessary <br />to maintain a permanent secure medicine take -back drop box. <br />(2) Disseminate public information including information on local treatment resources, naloxone <br />information and medical response cards and posters. (Print ready materials are available online <br />at www.stopoverdose.org). <br />Utilize publications already available through DSHS and other websites. (i.e., SAMHSA Opioid <br />Overdose Toolkit, and downloadable/printable materials on <br />http://www.dshs.wa.gov/dbhr/dapreventionpub.shtml#dbhr, www-stopoverdose-org and <br />www.ta ke backyau rmeds.or-g. <br />(3) Submit locally -developed educational and informational materials to DBHR for approval at least <br />ten (10) business days prior to publication. <br />DSHS Central Contract Services <br />1644CS Prevention Services - County (6-26-2015) Page 39 <br />
