See Company Spill Response Reporting, Loading/Unloading Policy, and Driver <br />Qualification policies on next page <br />b. If small spill occurs during loading of wastes, Thermo Fluids will generally <br />respond directly with additional driver and equipment resources or if spill is <br />larger than what Thermo Fluids can directly handle, Clean Harbors Field Services <br />Group can be mobilized to respond. The CHES Field Services Group's primary <br />business function is site clean-up and emergency response services. <br />Section B <br />1. Communication Plan— <br />a. Point of Contact for this contract during regular business hours (8:00 am <br />to 5:00 pm) for routine contract requests would be Amanda Mills, <br />Customer Service Representative. As back-up, local Account Manager, <br />Eric Parent is also available if needed. If TFI is awarded this contract, full <br />contact information for both TFI employees will be provided to the <br />Yakima County. <br />b. Any spills or other incidents that occur during service or transportation <br />activities conducted by TFI for the county will be reported within 24 <br />hours to Yakima County. Per company policy an incident form and <br />number is assigned to any spill, accident, or injury and this form can be <br />forwarded to county if needed <br />2. Certification of Disposal Documentation — At time of service proper waste pick-up <br />documentation is generated via TFI handheld point-of-sale device and is left with <br />customer representative. In the event no one is available on site to leave service <br />
