PROFESSIONAL SERVICES AGREEMENT <br />Between Kittitas County Jail <br />and Community Health of Central Washington <br />This Services Agreement ("Agreement") is entered into and effective as of this day of , 2025 <br />("Effective Date") by and between Kittitas County, a political subdivision of the State of <br />Washington, acting through its County Jail/Sheriff's Office ("County" or "Jail"), with offices at <br />307 W Umptanum Road, Ellensburg, WA 98926, and Community Health of Central Washington, <br />a Washington nonprofit corporation and a Federally Qualified Health Center ("CHCW" or <br />"Provider"), with its principal place of business at 501 S. 5«' Avenue, Kittitas, WA 98902. <br />1. Purpose <br />The purpose of this Agreement is to establish the terms and conditions under which CHCW shall <br />provide Substance Use Disorder (SUD) screening services to inmates held at the Kittitas County <br />Jail, in furtherance of public health goals and alignment with CHCW's federally approved scope <br />of project and Section 330 grant requirements. <br />2. Scope of Services <br />CHCW agrees to provide the following services: <br />• Initial SUD Screenings: Evidence -based screening for substance use disorders for <br />inmates within 14 days of intake. <br />• Assessment & Referrals: Clinical assessments for those screening positive and referrals <br />to appropriate follow-up services, including Medication -Assisted Treatment (MAT), <br />detox, or counseling. <br />Care Coordination: Referral to Connect and CHCW primary care providers or other local <br />treatment providers for post -release continuity of care. <br />Identify individuals at risk for TB or other infectious diseases and recommend follow up <br />with a provider for testing. <br />Documentation: Clinical documentation using CHCW's electronic health record system, <br />consistent with HIPAA and 42 CFR Part 2. <br />All services shall be delivered by qualified CHCW staff including, but not limited to, behavioral <br />health specialists, SUD professionals, and licensed clinical providers. <br />3. Term <br />This Agreement shall commence on the Effective Date and shall remain in effect for a period of <br />one (1) year, unless terminated earlier as provided herein. The Agreement may be renewed <br />annually upon mutual written Agreement of the parties. <br />4. Referral Workflow <br />To ensure continuity of care, compliance with federal grant objectives under Section 330 of the <br />Public Health Service Act, and alignment with Community Health of Central Washington's <br />(CHCW) integrated care model, the following referral workflow outlines the structured process <br />for identifying, assessing, and coordinating treatment for inmates with potential substance use <br />disorders.
