<br />Page | 32 <br />program entry. <br /> Targeted Prevention <br />HEN programs offering homelessness prevention assistance must prioritize households most likely to <br />become homeless and must use either the CHG Targeted Prevention Eligibility Screening Form or other <br />tool approved by Commerce. This form must be kept in the client file. <br /> <br />If modifying the CHG Targeted Prevention Eligibility Screening Form or using another tool, the risk <br />factors must be evidence informed. Submit the screening tool to your Commerce grant manager for <br />approval. <br /> Maintaining Homeless Status for Permanent Housing <br />While receiving Rapid Re-housing assistance, households maintain their homeless status for purposes <br />of eligibility for other permanent housing placements. <br />5.5 HEN Referral <br /> Documentation of HEN Referral8 <br />In place of income verification, lead/subgrantees must verify and document the household’s HEN <br />Referral from the Washington State Department of Social and Health Services (DSHS) as documented in <br />the Benefits Verification System (BVS) prior to program entry. <br /> <br />The CHG Verification of Household Eligibility and Income Recertification Form and HEN Referral <br />documentation must be kept in the client file. <br /> <br />For Pregnant Women Assistance (PWA) recipients with a HEN Referral, BVS will only display active PWA <br />households. If a household is no longer on the program (e.g. birth of baby), BVS will no longer display <br />an active status. Contact your Commerce CHG program manager to help determine PWA status. The <br />referral to HEN remains valid for 24 consecutive months. <br />5.6 HEN Referral Status Recertification <br />Lead/subgrantees must recertify a household’s HEN Referral from DSHS, as documented in the BVS, <br />every 12 months. The 12-month recertification timeline is based on the start date of rent assistance, <br />typically the month the household moves into housing. Both the HEN Referral and updated CHG <br />Verification of Household Eligibility and Income Recertification Form must be kept in the client file. <br /> <br />Recertification is not required for HEN households who are a PWA recipient up to 24 months. <br /> <br />Example Timeline: <br />✓ A household is determined HEN-eligible in May. <br />✓ The household moves into a housing and begins receiving rent assistance in July. <br />✓ The process would be: <br />• May: Initial eligibility is verified at the time of HEN referral. <br /> <br />8 HEN Referral includes households who have a HEN Referral and households enrolled in General Assistance (ABD recipient) and Ge neral Assistance <br />Pregnancy (Pregnant Women Assistance recipient) from DSHS as documented in BVS.
