5 <br />Special Terms and Conditions <br />Student Guide. <br />Statement of Work. The Contractor shall provide a program to provide the services, staff, resources, <br />and otherwise do all things necessary for, or incidental to the performance of work, as set forth below: <br />Services shall be sorted into the following three phases: <br />lntake and Assessment Phase. <br />Job Placement Phase. <br />Employment Phase. <br />a. lntake and Assessment phase. The intent of this phase is to ensure the Student and the Student's <br />Team collaborate to determine if enough information is available to establish an employment goal, <br />establish the employment goal, anO buitd an individualized roadmap of the Student's transition from <br />their education to m'eaningiul community employment. This shall be accomplished through the <br />following objectives: <br />(1) lntake <br />(a) At a minimum, the Contractor shallwork with the Student, the Student's legal representative <br />or guardian (unless they are their own), the CRP, and the DVR Counselor to: <br />i. Complete intake forms as needed, including the SDOP' <br />ii. Oufline how assessment will occur, including additional meeting time. <br />(b) The employment goal is established and documented' <br />i. Determine what Job Seeker Tools are needed for the Student's employment goal. <br />(2) Assessment <br />(a) Through the assessment process, the Contractor will collect information about the Student's' ' <br />strengtns, skills, and supports needed for success on the job. This information will be based <br />on the Student's school, home, and community work-related activities, interviews with the <br />Student, family, school personnel, and others who are familiar with the Student. This <br />information miy Oe obtained from Job Foundation report, work-based learning, school <br />assessments, etc. <br />(b) At a minimum, the following criteria must be addressed: <br />i. History of school attendance and reliability; <br />Behavior support or potential strategies;il. <br />iii <br />iv <br />V. <br />vi <br />Student Communication skills; <br />Student learning Preferences; <br />Student's personal hYgiene; <br />Potential job safetY concerns; <br />DSHS Central Contract Services <br />6017CF County Program Agreement (10-31-2017)Page 6
