6 <br />Special Terms and Conditions <br />(4) Higher Education, by exception approved by the.DVR Transition Program Manager or <br />designateJi"pr"i"rit"tive. Studeni starts School-to-Work services as of May 1,2026 or <br />later, as determined by the Student' <br />(a) School-to-Work is a wrap-around service that's only available one (1) time per Student' <br />i. Students who choose to participate in this cohort may not participate again under a <br />different contract covering a di*erent cohort. Should the Student change their mind on <br />the cohortihey choose tJparticipate in, the Contractor is responsible for communicating <br />with DVR to ensure there are no duplicate payments' <br />ii. should a student move from one school-to-work county to another, or pause services, <br />due to individual circumstances, it is determined by the DVR Transition Program <br />Manager or designated representative, whether School-to-Work services will continue, <br />and if so, at one point in the process' <br />(b) Students must apply for DVR to start School-to-Work services no later than January 31, <br />2027. Students who intend to start services after January 31 may only do so with prior <br />written approval from the DVR School-to-Work Transition Program Manager or their <br />designated rePresentative. <br />(5) Between ages 20-22. Students who have been identified to start services at age 19 <br />may Oo so-witn prior written approvalfrom the DVR School-to-Work Transition Program <br />Manager or designated representative' <br />(6) Have an lndividualized Education Plan (lEP)' <br />(7) Estabtish a DVR tndividual Plan for Employment (lPE) with an employment goalthat <br />requires youth Extended Services as needed and Long-Term Services in order to be <br />achieved. This requirement must be completed by the end of the School-to-Work lntake <br />andAssessmentPhasepersection5,statementofWork. <br />Meetings must be held in the format best suited for the Student. lf all parties are not able to attend <br />meetings in person, use of telephone or video conferences is encouraged to complete the process <br />in a timely manner. <br />Contractor must keep all Students enrolled in School-to-Work through the duration of this Contract, <br />regardless of outcome and/or Stabilization, unless Student chooses to exit the program of their own <br />volition or due to unforeseeable circumstances. <br />(1) Should a Student exit early for any reason,_Contractor must document the departure in <br />Exhibit D - School-to-Work Student Data. See Section 6, Deliverables for further <br />instruction. <br />Service Delivery Agreement: The Contractor's delivery of VR services specified in this agreement <br />will begin upon inegompletion and signing of the agreed upon SDOP. <br />Service Delivery payment: The Contractor will be paid upon completion and submission of the <br />required documentition demonstrating completion of each of the three phases for each Student as <br />outlined in Section 5, Statement of Work. <br />service Delivery Method: This contract utilizes Exhibit c - student Guide as a reporting tool and all <br />associated activities ouflined in section 5, Statement of work shall be carried out using the Student <br />Guide.
