Special Terms and Conditions <br />vii. Student stamina/endurance; <br />viii. Student social skills; <br />ix. Demonstrated skills and task performance; <br />x. Transportation needs; and <br />xi. Work preferences and interests. <br />(c) Based on the above criteria, the Contractor must determine recommendations regarding <br />appropriate and attainable job goal(s) that align with the Student's: <br />i. Skills and interests; <br />ii. Desired work environment; <br />iii. Desired schedule; <br />iv. Ability to travel to/from work within a reasonable length of time; and <br />v. Accommodations and supports necessary to achieve identified goals. <br />b. Job Placement Phase. The intent of this phase is to provide the Student with the tools they will <br />need to find and secure meaningful community employment based on their employment goal. The <br />Contractor shall provide all services necessary to locate, secure, and place a Student into <br />competitive integrated employment that is mutually agreed upon by the Student's Team. Specific <br />placement expectations shall be outlined on the DVR SDOP for Job Placement and Intensive <br />Training. <br />Placements less than 10 hours: Job placement goals that are less than 10 hours per week shall be <br />approved in advance by the appropriate DVR Vocational Rehabilitation Counselor in consultation <br />with the Student's Team. <br />(1) Job Seeker Tools <br />(a) Contractor shall create tools based on individual need, such as a resume, cover letter, <br />videos, PowerPoint (or comparable applications), and/or portfolios. <br />(b) Contractor may also develop other materials as it pertains to the Student's individualized <br />pathway to employment such as a letter of recommendation, create and/or update Linkedln <br />page or other social media, etc. <br />(2) Job Offer <br />(a) If the Student received the job offer, Contractor must provide details of the job offer including <br />the following: <br />i. The Student's Employment Goal; <br />ii. Job Title; <br />iii. Start Date; <br />DSHS Central Contract Services <br />6017CF County Program Agreement (10-31-2017) Page 7 <br />
