<br /> <br />Kittitas County <br />Cle Elum Transfer Station <br />Driveway Improvements and Tank Replacement Project Bid Documents <br /> <br /> <br /> <br /> <br />CERTIFICATE OF INDUSTRIAL INSURANCE PAID <br /> <br /> <br /> <br />Prior to project closeout the contractor shall: <br /> <br />Provide the Owner with the certification below. <br /> <br />Provide a copy of the employer’s prior quarterly records for the duration of the project. <br /> <br /> <br />Certification of Industrial Insurance Paid <br /> <br />I (Contractor) hereby certify that all industrial insurance has been paid to <br />the State of Washington as required by law for all work on this project including the work of <br />subcontractors and that the Kittitas County shall be indemnified and held harmless from any and all <br />claims arising from disputes over payment of industrial insurance with the State or any other person or <br />entity. <br /> <br /> <br />Signature Date <br /> <br /> <br />
