ATTACHMENT "A" <br />SCOPE OF WORK <br />ln addition to providing all material and labor, the Contractor shall perform the following activities as indicated in <br />your performance lndicators and Evaluation Workbook for the Behavioral Health Bridges project: <br />o Project Planning and Coordination <br />o Contract with behavioral health services partner and offer one-on-one counseling with ticensed <br />professionaI <br />r Build relationships and coordinate with community partners <br />o Provide Care Navigation and Case Management <br />o Conduct comprehensive mental health and needs assessrnents <br />r Make closed-loop referrals <br />o Develop personalized stability plans with clients <br />o Review and adjust clients'goals as needed <br />o Offer workshops and one-on-one support <br />o lnvolve certified peer support specialists <br />o Provide navigation and connection to resources <br />o Housing Access and Support Services <br />o Track and report on key metrics <br />o Review client outcomes and gather feedback <br />o Coordinate community engagement and increase awareness <br />. lmprove access to educationaI materiats <br />Kittitas County Agreement for Services (rev.5/14/25) <br />Page 3 of 23
