ATTACHMENT "A" <br />SCOPE OF WORK <br />In addition to providing all material and labor, the Contractor shall perform the following activities as indicated in <br />your Performance Indicators and Evaluation Workbook for the Behavioral Health Bridges project: <br />• Project Planning and Coordination <br />• Contract with behavioral health services partner and offer one-on-one counseling with licensed <br />professional <br />• Build relationships and coordinate with community partners <br />■ Provide Care Navigation and Case Management <br />• Conduct comprehensive mental health and needs assessments <br />• Make closed -loop referrals <br />• Develop personalized stability plans with clients <br />• Review and adjust clients' goals as needed <br />• Offer workshops and one-on-one support <br />■ Involve certified peer support specialists <br />• Provide navigation and connection to resources <br />• Housing Access and Support Services <br />■ Track and report on key metrics <br />• Review client outcomes and gather feedback <br />■ Coordinate community engagement and increase awareness <br />• Improve access to educational materials <br />Kittitas County Agreement for Services (rev. 5/14/25) <br />Page 3 of 23 <br />
