Kittitas County Health Network - THRIVE Youth Mentoring <br />Fu n di n g Re q u est: 53 79,805.90 <br />Average Score: L9.4/25 <br />KCHN submitted a grant application for the THRIVE Youth Mentoring program. The program is in <br />alignment with the Community Health lmprovement Plan. Using existing staff, the Kittitas <br />County Health Network has developed a youth mentoring program using the One-to-One <br />Community-Based mentoring model. With support from the county, the Project Specialist will <br />recruit, train, and match each mentor with one youth, resulting in at least 40 mentor-mentee <br />relationships by December 31st, 2021. <br />Why they are being recommended: They have proven to manage grant requirements and <br />funding well in the past and the community would benefit from the youth mentoring model <br />being brought back to our county. <br />Peers Rising - Warmth & Wellness: Resilience and Life Skills Support in Kittitas County <br />Funding Request: S 346,309.00 <br />Average Score: L9.2/25 <br />Peers Rising applied for funding to support the Warmth & Wellness Project. The project expands <br />access to prevention-focused behavioral health supports for youth and families in underserved <br />and rural areas of Kittitas County. Through peer-led youth groups and family workshops, the <br />project increases the diversity of services available outside clinical systems and strengthens <br />early intervention pathways. They will also offer coats, gloves, hats, socks, and hygiene items to <br />reduce barriers to participation and increase well-being and self-worth. Distribution will occur at <br />group sessions, workshops, and community events to ensure reach. <br />Why they are being recommended: They are a growing local organization that provides support <br />to a new population of kids and families, and they offered an estimate of youth to-be served <br />that demonstrates a large impact on our community.
