Kittitas County Health Network – THRIVE Youth Mentoring
Funding Request: $319,805.90
Average Score: 19.4/25
KCHN submitted a grant application for the THRIVE Youth Mentoring program. The program is in
alignment with the Community Health Improvement Plan. Using existing staff, the Kittitas
County Health Network has developed a youth mentoring program using the One-to-One
Community-Based mentoring model. With support from the county, the Project Specialist will
recruit, train, and match each mentor with one youth, resulting in at least 40 mentor-mentee
relationships by December 31st, 2027.
Why they are being recommended: They have proven to manage grant requirements and
funding well in the past and the community would benefit from the youth mentoring model
being brought back to our county.

Peers Rising – Warmth & Wellness: Resilience and Life Skills Support in Kittitas County
Funding Request: $ 346,309.00
Average Score: 19.2/25
Peers Rising applied for funding to support the Warmth & Wellness Project. The project expands
access to prevention-focused behavioral health supports for youth and families in underserved
and rural areas of Kittitas County. Through peer-led youth groups and family workshops, the
project increases the diversity of services available outside clinical systems and strengthens
early intervention pathways. They will also offer coats, gloves, hats, socks, and hygiene items to
reduce barriers to participation and increase well-being and self-worth. Distribution will occur at
group sessions, workshops, and community events to ensure reach.
Why they are being recommended: They are a growing local organization that provides support
to a new population of kids and families, and they offered an estimate of youth to -be served
that demonstrates a large impact on our community.
