Special Terms and Conditions <br />long-term supports in place <br />(b) Job stabilization is determined for each Student in supported employment based on their <br />unique circumstances. The DVR Counselor determines that the Student's work performance <br />is stabilized with input from the Student, employer and extended service provider when: <br />i. The Student has reached the agreed-upon goal for the number of weekly hours worked, <br />unless the Student and the DVR Counselor agree that the job represents substantial and <br />suitable employment. <br />ii. The Student has reached a maximum level of on{he-job employment and meets <br />employer expectations. <br />iii. The supports available are adequate for the Student to maintain stable employment and <br />meet the employer's expected level of work productivity. <br />iv. The Student has long{erm employment supports available or in process and is able to <br />transition to these services to maintain job stability. <br />(c) Once achieved, a Stabilization Report and supports on the job the student may need, must <br />be completed for the Student as outlined in the SDOP and must include: <br />Detail and summary of how the placement hours were met (or if the job represents <br />suitable employment); and <br />ii. Detail, descriptions, and summary of how the DVR Counselor, CRP, and the employer <br />agrees that the employer's expectation have been met; and <br />iii. Detail and summary of how longterm supports have been set up and will occur; and <br />iv. The strengths the Student brings to the job; and <br />v. How the Student will become stable on the job (i.e., their fading plan); and <br />vi. Additional training, services, and supports the Student may need. <br />(4) Extended Services <br />(a) DVR shall provide Extended Services needed by the Student until DDA funds are available. <br />i. Should Extended Services reach six (6) months, the Contractor and DVR shall <br />collaborate to determine next steps which may include additional Extended Services or <br />other options as needed. <br />(b) Extended Services funded by DVR shall be outlined on the DVR Extended Support SDOP. <br />i. Maximum 26 hours per month at the hourly rate of $105.00 <br />(A) lf more than 26 hours per month are needed for a Student, the DVR Counselor must <br />document the requirement for more service hours and staff the case with their <br />supervisor to request an exception to this maximum. <br />(B) Exceptions may be made on a case-by-case basis and must be reported to the DVR <br />School-to-Work Transition Prog ram M anager or designated representative. <br />DSHS Central Contract Services <br />60 1 7CF County Program Agreement (1 0-3'l -2017 )Page 9
