to the Kittitas County Public Health Departrnent any health-related problems discovered on routine <br />visits conducted by site monitors for Kittitas County Public Health Department follow-up, if necessary. <br />The Health District shall submit copies of the inspection(s) to <br />the invoice to OSPI by September 30, 2028. <br />both the sponsor and OSPI along with <br />PERIOD OF PERFORMANCE <br />Subject to its other provisions, the period of performance of this Agreement shall commence on the <br />date of execution, and be completed on September 30, 2025, f.rnless terminated sooner as provided <br />herein. <br />i <br />PAYMENT <br />i <br />Compensation for the work provided in accordance with this Agreement has been established under <br />the terms of RCW 39.34. The parties have determined that thelcost of accomplishing the work herein <br />will not exceed a total of one thousand nine hundred thirty-nine dollars ($1,939). Payment for <br />satisfactory performance of the work shall not exceed this amount unless the parties mutually agree <br />to a higher amount. Availability of funding may increase fhe ntlmber of inspections and increase the <br />value of the contract, through contract amendment initiated by OSPI. <br />Ir Eleven (11) health inspections at a rate of one hundred seventy-six and twenty-five cents <br />dollars ($1 76.25) per inspection/evduation. <br />One hundred percent (1OO%) of funds for the payment of this Cdntract are provided by federal program <br />Summer Food Service Program (SFSP), Catalog of Federal pomestic Assistance, CFDA #10.559, <br />therefore Health District shall adhere to Attachment A - FederallGrant Terms and Conditions. <br />BTLLTNG PROCEDURE i <br />I <br />Kittitas County Public Health Department shallsubmit invoices:to the OSPI Contract Manager, Petra <br />Colindres, no later than September 30, 2025, for those inspebtions/evaluations actually performed <br />for the SFSP. lnvoices received after September 30, 2025, WILL NOT BE PAID. The invoice shall <br />include the Agreement number and docum:nt to the Contract Manager's satisfaction a description <br />of the work performed, and payment requesied. Within approximately thirty (30) calendar days of the <br />Contract Manager receiving and approving the invoice,will be mailed or electronically <br />transferred to Health District by Agency Financial Services, O Upon expiration of the Agreement, <br />any claim for payment not already made shall be submitted <br />date or the end of the fiscal year, whichever is earlier. <br />n thirty (30) days after the expiration <br />lf errors are found in the submitted invoice or supporting ents, the Contract Manager will notify <br />Health District. ln order to receive paymenl, it shall be the nsibility of Health District to make <br />corrections in a timely manner, resubmit the invoice and/or s <br />and notify the Contract Manager. <br />AGREEMENT ALTERATIONS AND AMENDMENTS <br />orting documentation as requested, <br />This Agreement may be amended by mutual agreement of parties.
