ATTACHMENT"c, INSURANCE REOUIREMENTS Contractor shall secure and maintaln in effect at all thes during performance of work under this Agreement such Insurance as wlll protect contractor, Its employees, and agents from all clalms, losses, harm, costs, llabllltles, damages and expenses arising out of Contracto/s performance under this Agreement, lncludlng but not limited to personallnJury {including death) or property damage. All insurance shall be lssued by companies admifted to do business ln the State of Washlngton and have a ratlng of A-, ClassVll orbetterin the most reently publlshed edition of Best's Reports unlessotherwise approved by the County. lf an insurer ls not adrnitted, all insurance pollcies and procedures for lsulng the insurance pollcles must comply with Chapter48.15 RCW and 284-15 WAC. At a mlnlmum, C.ontractor shall malntain and provlde proof of the followlng selected options! Workers' Compensation & Emplove/s Llabllltv. Contractor shall provlde Workers Compensatlon and Employe/s Llability insurance on a state-approved pollcy forrn providing beneflts as requlred by law wlth employey's llablllty llmits no less than $1,000,000 per accident or disease. Professional Llability / F,rlorc and Omlssions liabllltvr Coverage limits not less than:r $1,000,000 each claimr Contrdctor must provlde evldence of this coverage on a pollry form appropriate to Contractoy's profieslcn. Contractor shall fumish to the County a Certiflcate of lnsurance, as evldence that policles provlding insurance requlred by this Agreement are in full force and effuct Contracto/s lnsurance policies requlred above must apply on a prlmary non-contrlbuting basls ln relatlon to any other insurance or self-lnsurance available to the County. Contractor agrees to provue notlce to the County at least thlrty (30) days prior to cancellation, or any materlal alteratlon or non-renewal, of any of the above-requlred insurance coverages. Contractor shall assume full responsibility for all loss or damage from any cause whatsoever to any tools, machinery equlpment, or motor vehicles owned or utlllzed by Contractor. or Contracto/s agents, employees, suppliers or contractors, as well as to any temporary structures, scaffolding and/or protectlve fences. Contractor shall have sole responslblllty fior ensuring the lnsurance coverage and llmlts requlred hereln are also obtalned by any subcontractors.
