LjVUUOly II r-I Ivcluyc I U. 4wv4.7o I-J I Jr-40VL-D0DI-1 IC Iro'+DU4 I <br />State of Washington, Department of Ecology <br />IAA No. C2500178 <br />Entity Name: Kittitas County Public Health <br />• CONTRACTOR assists businesses as needed with paperwork required to apply for <br />reimbursement, including a state payee registration form. <br />■ CONTRACTOR must communicate to the businesses that it may take up to 4 months to receive <br />payment from ECOLOGY after purchase and that the business must respond to inquiries from <br />ECOLOGY or the Office of Financial Management (OFM) in a timely manner to avoid delays in <br />payment. <br />■ Business purchases approved product or equipment and converts fully to utilization of new <br />product or equipment in accordance with the eligibility criteria for the PRP reimbursement. <br />+ Business submits receipts for the product or equipment purchase and installation to ECOLOGY's <br />PRP Coordinator.
