'TUJu•Tuu I -J / Jr-4ow-.7oo 1-1 1 C I ru4C.74 I J <br />State of Washington, Department of Ecology <br />IAA No. C2500178 <br />Entity Name: Kittitas County Public Health <br />The CONTRACTOR is expected to: <br />• Interact with other partners within the PPA Partnership to provide technical assistance and <br />training and share resources and experiences. <br />• Set up alerts to receive notifications when requests for information have been made on the PPA <br />Partnership SharePoint Discussion Board. <br />• Ensure at least one staff member is available to provide timely information and feedback to <br />ECOLOGY's PPA Coordinator and to attend mandatory meetings and trainings. Feedback on <br />Partnership goals, direction, and projects will occasionally be requested via online surveys and <br />email requests. <br />■ Act in a professional and ethical manner and avoid any conflict of interest that might influence <br />the CONTRACTOR's actions or judgement. <br />• Disclose immediately to ECOLOGY any interest, direct or indirect, that might be construed as <br />prejudicial in any way to the professional judgment of the CONTRACTOR in rendering service <br />under this Agreement. <br />• CONTRACTORS will attempt to provide 40% of their services (visits and unique program <br />elements) to businesses located within overburdened communities of Washington State. The <br />Overburdened Communities of Washington State map provided by the Office of Financial <br />Management is used to determine if businesses are located within an overburdened community.' <br />• CONTRACTORS are required to identify facilities within Overburdened Communities in <br />Washington State during data entry into the Local Source Control Database. <br />Key staff, estimated FTE, and their roles are identified in Table 1. Please note, this is an estimate of time <br />dedicated to this contract over the full two years of the contract; quarterly invoicing must reflect actual <br />hours worked even if hours are higher or lower than the FTE estimate. <br />Key personnel changes (staff or manager leaving, new hires, etc.) must be reported to the Pollution <br />Prevention Assistance Coordinator within ten (10) business days. Technical Assistance Visits <br />The CONTRACTOR will conduct technical assistance site visits to small quantity generators of <br />dangerous wastes, and to businesses or organizations that have the potential to pollute stormwater. <br />Approximately 60% of the visits will be Initial Visits. If Initial Visits fall below 60%, combined Initial <br />Visits and Follow -Up Visits must account for at least 80% of the total visits.
