EXL|IF|T'W' <br />SCOPE OF WORK <br />CompaseDircct Healthcare intendsto provlde a ModlcalAsslstant (MA) in the Ktttitas <br />County Jall 5 days a week, 40 houre a weekto asslst with the Washington State <br />Healthcare 1115 reentry initiative. The MAwlllwork wlth all prognamming related to <br />reentry/medlcal/mental health/behavloral health qs well as outslde entlties providing <br />reentry targeted oase management as required as part of the l1'15 initiative. ln the <br />event of the MA belng on leave, both planned and emergent, CompassDlrect Healthcare <br />wlll be responsible for covering the hours with a replacement staff member during thot <br />same time. <br />The MA will do as$essments, Medicaid ellgibillly a$$e$$ments, communicate with <br />programming, work clooely wlth the Kittitas County Jail staff to navlgate cllents, set up <br />the appolntmente both in-person and telecommunicatlon for cllents and programmlng, <br />process and retain papennrork ao requlrcd, and work closely with the Kittitas County Jall <br />leadershlp to enhance and sustaln 1115 and reentry progmmmlng. <br />Goals and ObJectlves <br />1. Provide a Medical Assigtant to work in the Kittltas Gounty Jail for 5 days a woek, <br />40 hours a week <br />2. To work as part of a larger team of program staff focused on the Washlngton <br />1{ 15 reentry lnitlative for lncarcorated populatlons <br />3. To ensure that all requirernents of 1115 reentry lnitiative aro being met <br />Proposed Frogram <br />To accompllsh the above-described objectives, CompassDirect Healthcare intends to <br />provlde an MA to work with the Klttltas County Jail staff and otherprovlders on all <br />necessary requirements of the 1 1 15 reentry initiatlve and the care for those <br />incarcerated at the Klttitas County Jail. This lncludes, but ls not timited to, developlng <br />proces$es and procedures to ensure every lndividual who is booked and housed at the <br />Kittitas County Jail is assessed for Medlcaid eligibility, enrollrnent or reenrollment, or <br />private lnsurance wlthin 24 houre of inlake (baoed on a client's ability and wlllingness to <br />provide appropriate lnformation) and submlts a completed and signed application for <br />Apple Health insurance wlthin 2 business days. The MAwillthen workwlth allasslgned <br />Manage Gare Organizatlons (MCOs) and their Reentry Targeted Case Management <br />(rTCM)team to navigate clients through necessary care, appolntments, paperwork, and <br />planning towards reentry and at time of release. <br />The MA willslso take the necesehry steps and malntain access to Washington Health <br />PIan Flnder, Provicler One, and all related programs to accurately enter, malntain, and <br />Professlonal $ervlces Agreernent (rev. 0gl24l2o18l <br />Page 13 of t8
