EXHIBIT "A" <br />SCOPE OF WORK <br />CompassDirect Healthcare intends to provide a Medical Assistant (MA) in the Kittitas <br />County Jail 5 days a week, 40 hours a week to assist with the Washington State <br />Healthcare 1115 reentry initiative. The MA will work with all programming related to <br />reentry/medical/mental health/behavioral health as well as outside entities providing <br />reentry targeted case management as required as part of the 1115 initiative. In the <br />event of the MA being on leave, both planned and emergent, CompassDirect Healthcare <br />will be responsible for covering the hours with a replacement staff member during that <br />same time. <br />The MA will do assessments, Medicaid eligibility assessments, communicate with <br />programming, work closely with the Kittitas County Jail staff to navigate clients, set up <br />the appointments both in -person and telecommunication for clients and programming, <br />process and retain paperwork as required, and work closely with the Kittitas County Jail <br />leadership to enhance and sustain 1115 and reentry programming. <br />Goals and Objectives <br />1. Provide a Medical Assistant to work in the Kittitas County Jail for 5 days a week, <br />40 hours a week <br />2. To work as part of a larger team of program staff focused on the Washington <br />1115 reentry initiative for incarcerated populations <br />3. To ensure that all requirements of 1115 reentry initiative are being met <br />Proposed Program <br />To accomplish the above -described objectives, CompassDirect Healthcare intends to <br />provide an MA to work with the Kittitas County Jail staff and other providers on all <br />necessary requirements of the 1115 reentry initiative and the care for those <br />incarcerated at the Kittitas County Jail. This includes, but is not limited to, developing <br />processes and procedures to ensure every individual who is booked and housed at the <br />Kittitas County Jail is assessed for Medicaid eligibility, enrollment or reenrollment, or <br />private insurance within 24 hours of intake (based on a client's ability and willingness to <br />provide appropriate information) and submits a completed and signed application for <br />Apple Health insurance within 2 business days. The MA will then work with all assigned <br />Manage Care Organizations (MCOs) and their Reentry Targeted Case Management <br />(rTCM) team to navigate clients through necessary care, appointments, paperwork, and <br />planning towards reentry and at time of release. <br />The MA will also take the necessary steps and maintain access to Washington Health <br />Plan Finder, Provider One, and all related programs to accurately enter, maintain, and <br />Professional Services Agreement (rev. 09/24/2018) <br />Page 13 of 18 <br />
