Ptease submit progress reports to the cterk at <br />heaLthandhumanser-vie es@e o.kiltitas-.wa.u-s by the tast day of the month fottowing the <br />quarter in which services were delivered. <br />The report shal,l incl,ude, at a minimum: <br />o A narrative of project activities during the reporting period, inctuding the <br />activities outtined in Exhibit A. <br />o A narrative of project successes during the reporting period. <br />o A narrative of project chattenges during the reporting period. <br />o A narrative of progress made toward the project objectives in Exhibit A. <br />r A data summary of the fottowing outputs: <br />o For lncreasing Prevention and Enhancing Access to Care and Services: <br />. Number of individuats receiving or participating in evidence-based <br />behavioraI heatth-retated services as a resutt of the grant <br />. Number of individuats trained in prevention or behavioral heatth <br />promotion <br />. Number of individual,s trained in suicide risk assessment as a resutt of <br />the grant <br />. Number of individuats screened for suicide ideation as a resutt of the <br />grant <br />.
