COI'AMU NITY PROTECTI ON WAIVER <br />This waiver is for people who need Z4-hour on-site awake staff supervision and therapies to maintain <br />their own and community safety. Services covered include: Residentiat Services, Behavior Support, <br />Therapies, EnvironmentaI Adaptations, Skitled Nursing, Emptoyment services, Speciatized Medicat <br />Equipment and Suppties and more. <br />INDtvtDUAL & FAtAtLy SERVTCES WATVER (tFS) <br />This waiver became effective June 1 , 2015, and offers support to ctients tiving with a family member in <br />the community. The intent of this program is to provide support and services to famity members, <br />centered on the needs of the ctient and the family. Services covered include the fottowing:r IFS funding levels range from 51,200 to 53,600, depending on assessed need (determined by <br />assessment). <br />' IFS services include Respite; Behavior Support and Consuttation; Community Engagement; <br />Home and Vehicte Modifications; Peer Mentoring; Assistive Technotogy and Speech; <br />Occupational, and PhysicaI Therapies; Person Centered Ptan Facititation; Speciatized ctothing <br />and nutrition; skitted Nursing; and Transportation to a waiver service.r IFS services can be used in any combination, not to exceed a ctient's annua[ alocation. <br />'HILDREN'S <br />/NrENs/yE |N-HO/I[E BEHAVTORAL SuppORT (CilBS) <br />This waiver combines wrap-around ptanning with positive behavior supports to empower famities to <br />remain together. To be etigible for CllBS, your chitd must be between the ages 8 through 20 years otd <br />living with their famity and are assessed at high or severe risk of out-of-home ptacement due to <br />chatlenging behaviors. Services covered are determined by the lndividuat Support Ptan with an average <br />cost of service, inctuding respite, of 54,O0O per month per ctient for any combination of the fottowing <br />services: Skitted Nursing, Nurse Delegation and Behavior Support and Consultation; Environmental <br />Accessibitity Adaptations; Assistive Technotogy; Therapeutic Equipment and Suppties; Speciatized <br />Medical Equipment/Supplies; Speciatized Nutrition and ctothing; Vehicte Modifications; Occupational <br />Therapy; Speciatized Psychiatric Services; Physicat Therapy; Speech, Hearing and Language Services; <br />Staff/Famity Consuttation and Training; and Transportation. <br />ACCESSING WAIVER SERVICES <br />STEP 1 <br />ln order to access county funded services, individuats with devetopmental disabitities must be <br />determined etigibte for services through the DevetopmentaI Disabitities Administration (DDA). The assessment <br />determines what services a person may be etigibte for. lf certain criteria are met, a request to be <br />placed on a Waiver Program can be made.
