Special Terms and Conditions <br />(a) For fiscal year 2021 911100 through 8/31/01 <br />(b) For fiscal year 2022 9/1/01 through 8131/02 <br />(c) For fiscal year 2023 9/1/02 through 8/31/03 <br />(d) For fiscal year 2024 9/l/M through 8/31/04 <br />These students currently attending school and have completed an application to participate in <br />this Value Based Payment (`vPB) project. The VBP projact application will include the following <br />minimum criteria identified in the sample application found at: <br />hU s://www.dshs.wa. or/sile�/default/files/DDA/dda/documeris/Job°/o20Foundation%20A lira <br />Lion 040720%2(1°/e28002%29.docx <br />(2) Qualified provicers will collaborate with School District staff to complete the Job Foundation <br />document and U-en prov de a copy to the County. To be a qualified provider for the Partnership <br />Project the Courties must require the provider to have a current contract with the Division of <br />Vocational Rehabilitation; a contract in good standing wi-h the County and cannot be in <br />provisional status; a minimum of two years' experience provid ng Individual Employment with <br />demonstrated job placement Skills. The Job Foundation -ecommendation will only have <br />actionable next steps towards integrated competitive employment. <br />(3) County participating in the Partnership Project must have the ability to work collaboratively with <br />School Districts to identify DEA students in high school, collect and track information, be <br />proficient in electronic spreadsheets, and ability to evaluate if completed Job Foundation <br />document(s) meet quality standards. <br />u. Employment Outco-ne Payment. —Providers, with transition students born between: <br />(1) For fiscal year 2023 9/1/00 through 8/31101 <br />(2) For fiscal year 2C24 911/01 through 8131/02 <br />(3) For fiscal year 2C25 9/1/02 through 8/31/173 <br />(4) For fiscal year 2C26 9/1/03 through 8/31/04 <br />High acuity Clients vvho are authorized for Individual employment, may receive one outcome <br />payment if student obtains a competitive integrated job approved by the County within timeframes <br />described in the chart in the DDA Billing Instructions. If the County is also the service provider DDA <br />will provide the job approval. If the job is at a minimum of ten hours of work per week an additional <br />amount will be include in the paynent. <br />7. Consideration: <br />a. Fees: <br />(1) Approval of fees is the responsibility of the DDA. The DDA HQ Employment and Day Unit <br />Manager reserve3 the right to approve fees/rates for the services being provided. The County <br />will submit a fee'rate schedule with the initial Program Agreement. The County will submit <br />updated feelrate Schedules to the HQ Employment and Day Unit Manager for approval as <br />changes occur.
