L4.3 <br />14.4 <br />D.The employee has not received a total of more than 960 hours of shared leave while employed by <br />Kittitas County; and <br />E. The employee is not receiving time-loss payments as a result of an on-the-job injury. <br />Eliejbilitv for Donatine Shared Leave: An employee may donate accrued leave to the Shared Leave bank <br />or to a specific eligible employee under the following conditions: <br />A, The employee is a regular paid employee who is covered under this Agreement and has <br />successfu lly completed their probationa ry period' <br />B. The employee has not been intimidated, threatened, or coerced into donating accrued leave for <br />the purposes of leave sharing. <br />14.3.L An employee who would like to donate leave must submit a "shared Leave Donation Request <br />Form" to the Human Resources Department. Leave may be donated as follows: <br />A. When donating vacation leave, the employee's accrued leave bank may not fall below 40 <br />hours of accrued leave after the donated leave has been subtracted. <br />B. When donating sick leave, the employee's accrued leave bank may not fall below 120 <br />hours of accrued leave after the donated leave has been subtracted' <br />C. When donating a Personal Holiday, the Personal Holiday must be donated in full-day <br />increments. <br />D. Leave may be donated to the Shared Leave Bank to be used for any approved request for <br />shared leave, or to a specific employee. <br />E. An employee may donate a maximum of 40 total hours per qualifying event. <br />An employee who would like to donate leave must submit a "shared Leave Donation Request <br />Form" to the Human Resources Department. The Human Resources Department will review the <br />donation request form for eligible and compliance with this Article. The employee and the Sheriff <br />will be notified if the request is approved or denied. <br />The name of the person donating leave will be kept confidential if requested. <br />Requesting and Using Shared Leave: An employee who would like to request shared leave must submit a <br />,,Shared Leave Request Form" to the Human Resources Department. lf the need for leave isforeseeable, <br />the employee must submit the request form at least thirty (30) days prior to the first day of leave' lf the <br />need for leave is not foreseeable, the employee should complete the request form as soon as possible and <br />practical. ln most cases, the County will requlre a medical certification to support a request for leave. lf <br />the employee has recently completed a medical cerlification for FMLA or PFML leave purposes, such <br />medical certification may be used to support the need for shared leave'
