One signed <br />copy of the Certification page shall be included with the bid. The inclusion of this Addendum will be <br />noted in the Agreement, and the execution of the Contract shall indicate full acceptance by all parties of <br />the provisions contained herein. Allother provisions of the Plans, Specifications, and other Contract <br />Documents not noted in the Addendum shall remain unchanged and in full effect.
