Furthermore, any changes to medications, questions about care, or discussions needed <br />to be had about the care for a certain patient will be communicated through CDH and <br />either the clients primary care giver when not incarcerated or the existing mental health <br />nurse practitioner. There may also be collaboration between CDH and KCJ mental <br />health staff required during this contract. <br />Research and Evaluation <br />CDH will evaluate the success of the KCJ behavioral/mental health practitioner and <br />Clinical Staff successes by tracking data associated with these services. This will <br />include the number of inmates seen, the total number of inmates treated, and perhaps <br />most importantly, the recidivism rate for inmates who were treated with <br />behavioral/mental health medications while in custody at KCJ. KCJ will gather and <br />analyze this data with quarterly and annual reporting available, unless othenryise <br />requested by KCJ. Any additional specific metrics KCJ desires to be tracked may be <br />requested as well. <br />KCJ is the owner of the research data, to the extent consistent with and allowed by the <br />grant funding and may make use of the de-identified data for the education of the public <br />and other entities. <br />At least quarterly a representative from CDH will be invited to participate in a meeting <br />and share any pertinent information regarding successes or barriers of the <br />programming, how the parties can improve on processes or communication, and <br />present information as requested.
