EXHI rrArr <br />SCOPE OF WORK <br />CDH intends to provide 24171365 access to medically trained personnel to provide <br />prompt access to treatment behavioral and mental health services. Medical staff will be <br />in the facility 5 days a week proving addition psychotropic prescriber time as well as 40 <br />hours a week Clinical Staff Coverage to what KCJ already has in place. ln the event of <br />the Clinical Staff being on leave, both planned and emergent, CDH will be responsible <br />for covering the hours with a replacement staff member during that same time. <br />Jail staff and other medical providers will always have access to CDH prescribers via <br />telephone. CDH staff will screen and triage fresh intakes/bookings and all other <br />incarcerated people for their needs as related to behavioral/mental health care as well <br />as monitor medications for follow-up, education, and reordering as necessary to ensure <br />the patient doesn't run out. <br />Goals and Objectives <br />L. To extend weekday access to behavioral and mental health services <br />z. Provide a Clinical Staff Member to work in the Kittitas County Jail for 5 days a <br />week, 40 hours a week <br />3. To provide 2417 on call services for emergency situations related to <br />behavioral/mental health medications <br />4. To ensure continuity of care amongst providers of care, collaborate with internal <br />and external stakeholders, including the existing mental health nurse practitioner <br />and therapist, via phone and in person as needed <br />Proposed Program <br />To accomplish the above-described objectives, CDH intends to extend their current AM <br />sick call hours as needed to promptly address access to behavioral and mental health <br />issues. lf the ability to not address all necessary patients during regularly scheduled <br />sick call hours, CDH will work with KCJ staff to determine time later in the day to return <br />to the facility and address any additional clients as needed. <br />The Clinical Staff will oversee triaging all mental health related patients and determine <br />which mental health practitioner they need to be seen by. The Clinical Staff will also be <br />responsible for medication passes, monitoring of medication levels, and setting the <br />appropriate follow-up needs for those they are providing services to. Any and all <br />necessary communication for clients and patients can be handled and navigated by the <br />Clinical Staff overseeing the mental health portion of services provided by CDH. <br />All complex behavioral/mental health care will be referred to the existing mental health <br />nurse practitioner to ensure complete and proper continuation of care for the patient. <br />Professi onal Servi ces Ag reem ent (r ev . 09 I 24 120 1 8) <br />Page 13 of 18
