EXHIBIT "A'' <br />SCOPE OF WORK <br />A. The Contractor shatt receive SFY 2025 SupplementaI Consotidated Hometess Grant <br />funding to fund hometessness crisis response systems and to assist people who are <br />experiencing or are at risk of hometessness to obtain or maintain housing in both <br />Adams County and Kittitas CountY. <br />B. The Contractor shatt commit to operating a high-performing crisis response system in <br />both Kittitas County and Adams County by: <br />a. Assessing each househotd's housing needs and facititating housing stabitity with <br />the goat of obtaining or maintaining permanent housing. <br />b. Emptoying a progressive engagement service modet. <br />c. Prioritizing househol,ds most tikety to become hometess when using homelessness <br />prevention rent assistance. <br />d. Being anti-racist [e_aders in their crisis response systems and facititate partnerships <br />among organizations that respond to the disproportionatity in services and <br />outcomes for communities that may not seek assistance f rom mainstream <br />organizations. <br />C. The Contractor shatt compl,y with a[[ requirements, poticies, and procedures in the <br />ConsoLidated Hometess Grant Guidelines as found in Exhibit D, inctudingthe <br />Washington State Coordinated Entry Guidetines, for atl use of funds under this <br />agreement. <br />D.
