Task <br /># <br />Activity Deliverables/Outcomes Due Date/Time Frame Payment Information <br />and/or Amount <br />Priority populations for Syringe Services Programs include <br />people who use drugs, with a focus on: <br />. People systemically marginalized and underserved <br />due to racism - Black/African American, <br />Lati no/Latina,/Latine/Lati nx, American <br />Indian/Alaska Native people and other <br />communities for whom there are documented <br />health disparities in your region. <br />. People who are unhoused or unstably housed. <br />. People engaged in sex work. <br />o People involved in the criminal legal systern. <br />. Genderexpansive/transgenderindividuals. <br />. Gay, bi, and other men who have sex with men. <br />NOTE: See Special Requirements, T€rms and <br />Conditions - Section 4 Syringe Services Programs: <br />Support for Operations Program Requirements for <br />additional task information. <br />Syringe Services Programs: Harm Reduction Service <br />Navrgation <br />Provide appropriate referrals to SSP participants; facilitate <br />access to receive health care and medical services, social <br />services, behavioral health counseling and other services <br />including substance use treatment (including medicatlons <br />for opiate use disorder, or MOUD); housing; and advocacy, <br />including but not lirnited to criminal legal involvement, <br />medical providers, benefits navigation, and family <br />reunification. <br />NOTE: See Special Requirements, Terms and <br />Conditions - Section 5 Syringe Services Program, Harm <br />Reduction Care Navigation Requirement for additional <br />task information. <br />Harm reduction care navigation outcomes <br />include delivering services and tracking: <br />. number ofpartrcipants enrolled in <br />care navigation services <br />. nurnber ofcare navigation sessions <br />. number ofrefenals to health and <br />social services <br />. number of linkages to care for health <br />and social services <br />. number ofoutreach attempts per <br />participant <br />DOH Program and Fiscal Contact Information for all ConCon SOWs can be found on the DOH Finance SharePoint site.
