GOALS AND ECTIVES FORM <br />Fill out white sections only. <br />GOAL: <br />To provide a warm, safe, and stable place for homeless individuals to shelter overnight <br />from November 15 - March 15 <br />Measures (How will you know you are making progress toward your goal?l Data Source <br />Document the number of new/return guests sheltered nightly HopeSource & <br />Strategy f1: <br />(What will you do to achieve your goal?) <br />Secure location(s) for the Shelter <br />Measurable Objectives <br />Commitments from 7 churches/organizations to <br />host the CWS 1 night/week Nov.15 2023 through <br />March 15 2024 and hotel rooms for 5 holidays will <br />be obtained. <br />Strategy f2: <br />(What will you do to achieve your goal?) <br />Attain required people. hire shelter coordinator, <br />coordinator assistant, and night watch. <br />Measurable Objectives: <br />A coordinator, assistant, and night watch persons <br />will be hired and HopeSource staff for data input <br />into the HMIS system will be organized for <br />program implementation. <br />Strategy f3: <br />(What will you do to achieve your goal?l <br />Recruit volunteers and obtain equipment/materials <br />to implement program safely and efficiently. <br />Measurable Objectives: <br />Volunteer lists and network within churches, <br />community organizations, and social media to <br />recruit volunteers, meals, hotels, and laundry <br />services will be completed.
