(Select all that apply and give <br />a brief 100 word description for each selected.) <br />|7[uickly identify and engage people experiencing homelessness. <br />Outreach and identification are accomplished through field visits to all areas and businesses <br />where homeless youth are likely to spend time. We track all outreach efforts weekly to ensure <br />anyone experiencing homelessness is identified and given continued opportunities to receive <br />assistance. We work closely with the City Homelessness Task Force to come alongside <br />individuals identified as homeless with high vulnerability and currently unengaged needing <br />services. lntakes include an internal assessment designed for each demographic we serve to <br />determine each client's levelof vulnerability and to develop a stability plan best suited to the <br />needs of each client. <br />r'Prioritize housing for people with the greatest needs <br />Literally homeless households receive priority over households that are imminently at risk. The <br />CE system, operated by HopeSource, has all households complete a standard assessment <br />survey. Tools and assessments are designed to be easilv understood by youth and to reflect the <br />participants' strengths, goals, risks, and protective factors. <br />r'operate an effective and efficient homeless crisis response system that <br />swiftly moves people into stable permanent housing. <br />The Kittitas County Continuum of Care Coordinated Entry System for homeless individuals has <br />been created to better maximize community resources and quickly re-house homeless <br />individuals or prevent eviction. Using a web-based entry system that can be accessed from any <br />computer or phone, the system is positioned to deliver information to the entire housing <br />provider network immediately in one step, avoiding the need to go "door to door" for <br />assistance. Agencies then communicate among themselves to determine the applicant's best <br />choice and prioritize services. The client is then contacted for a full intake with the best agency <br />to meet their needs. <br />o
