(250 words max.) <br />Key service providers that HopeSource regularly collaborates with in meeting the needs <br />of vulnerable populations in our community and in this property include: Central <br />Washington Healthcare which provides mental health supportive housing for individuals <br />coming out of therapeutic treatmenU WorkSource employment specialists; DSHS-SNAp <br />and temporary financial benefits provide security as clients construct their safety net; <br />the Social Security SOAR program that assists in moving from disability to ability; the <br />Ministerial Association which offers spiritual counseling and assists with small essential <br />needs; centralTransit, Hopesource Dial-A-Ride, and The Apple Line which provide <br />transportation services for the community to mitigate the expense of traveling in and <br />outside the county; Aging and Long-Term care and people for people which provide <br />aging in place and support for seniors; Kittitas County Commissioners who provide <br />funding for homeless and at-risk seniors over the age of 65; central washington <br />Disability Resources and Elmview who provide training and disability support; FtSH <br />which offers food and social support; the Free Clinic Dental & Vision that provide access <br />to medicaltreatmenU Goodwill and the Methodist Clothing Bank which provide basic <br />clothing and household needs; and the Kittitas County Health Network that encompasses all <br />medical, mental health, and social determinants of health providers in the county. <br />I
