EXHIBIT "A'' <br />SCOPE OF WORK <br />The Contractor shall employ one or more Community Health Workers / promotoras de <br />salud, who will be a member of the Kittitas county lncident Management ream <br />(lMT) and report to the Operation Sections Chief, following the lMf structure. Job <br />description is as follows: <br />The coVlD-19 Vaccine outreach community Health worker (cHW) is a temporaryparttime position expected to last approximately three (3) months. The employee in thisposition wilt specifically perform work with the Kittitas County lncident Management <br />Team (lMT) related to decreasing barriers to accessing COVID-1g vaccination for <br />Kittitas County's Latinx and Spanish-speaking households. The position requires <br />courteous and cooperative interaction with alldepartment and IMT staff, County, State, <br />and Federal agencies, and the general public, and will be expected to provide <br />assistance in other areas of Public Health as needed. All employees work under the <br />guidance of local/state/federal regulations. <br />IMPORTANT lhis is posifibn is expected to end Juty 26, 2021. Duties and hours may <br />be amended as determined by COVID-I9 funding availabitity. <br />Essential Functions:. lmprove COVID-19 testing and <br />Spanish-speaking comm un ities <br />vaccination access and uptake in local Latinx and <br />by: <br />' ldentifYing and implementing outreach strategies to reduce barriers to testing and <br />vaccination. lncreasing community COVID-19 knowledge <br />' Developing effective relationships with community members and community <br />partners <br />. Assisting with appointment scheduling <br />OTHER FUNCTIONS: <br />. Res pond to Public Health related inquiries from the public as neededr Maintain confidentiality as described in agency, federal, and state required policies. <br />This iob description is a summary of the primary functions and expectations for this <br />position.
