5. Purpose of Use: Classroom for KCPHD/U\I/ research project. <br />6, Rent: $300/rrrurrtlr <br />7. Special conditions: KCPHD will be responsible for cleaning the classroom. Termlnation: <br />This agreement shall cgntinue for the agreed lerm, or untileither party terminates this <br />agreement by giving at least thirly (30) days written notice. User shallvacate the <br />Premises imrnediately upon termination of tlre agreement. <br />9. Notices and Contact lnformation: <br />All notices pertaining to this agreement shall be made in writing to the parlies <br />at the lollowing addresses and phone numbers: Church otfice (EUMC, Z10 N. Ruby <br />Street, Ellensburg, WA 98926, 509-962-9257). Building Use Managers: <br />(509-899-5028, Nancy Goodloe) or (509-607-1342, Karen Lasik). <br />10. Conditions of Use: <br />a. Rental Fees shall be submitted by the 1st week of every month or the comple- <br />lion ol the contractualterms above. Please remit to paymenls to EUMC in care of <br />Bill tseals, lreasurer. <br />b. Church policy requires no smoking or firearms on the premises. <br />c. Alcoholic beverages shall not be brought onto the premises unless the user <br />group has been given permission well in advance ol the event from the Buildinq <br />Use Managers, <br />d. The premises shallbe a violence-free zone. <br />e. User shallnot discriminate on the basis of gender, race, age, sexualorientation, <br />national origin, mental or physical disabilities, marital slatus or economic condition. <br />f. All children within the church building or grounds MUST be supervised by the <br />User group and not allowed to be in any areas of the building or its premises with- <br />out an adult supervisor. This practice is consistent with the "Safe Sanctuaries" for <br />children and youth policy adopted by this congregation. <br />The following guidelines of the Safe Sanctuary policy are particularly important: <br />i. The Safe Sanctuaries policy typically requires no fewer than two adults to be <br />present at alltimes during a program or event involving ohildron. The Kittitao <br />County Public Heallh Department is authorized to have a single staff person with <br />all required state background checks overseeing the children of clients without a <br />second adult present. <br />ii. At least one of the adults present should be competenVcurrently certifiod in <br />First Aid and CPR. <br />iii. The program staff rnust readlhe$afe Sanctuary policy and agree to its terms <br />as mandated reporters of child abuse. lf a case ol child abuse is observed by <br />KCPHD Staff within their group, or by other groups on the church premises, while
