TERMS AND CONDITIONS <br />Thb CONTRACT is entered hto <br />(WASPC);and <br />by and between the WASH(f tr INGTON ASSOCIATION OF <br />(coNTRACTOR)SHERIFFS & POLICE CHIEFS [.,]'tr Lr' <br />NOW THEREFORE, in consideration of the covenants, performances, and promises <br />contained herein,the parties agree as follows: <br />{. FUNDING SOURCE. Funding for this CONTRACT is provided to WASPC by theWashington State Department of Health, through theAmerican Rescue plan Act of iO21,P.L. 117-2 and under the Detection & Mitigati6n of COVID-19 in Confinement Facilitiesproject, was awarded to WA DOH in September ?O2L The pass-through allocation forthe administration of the grant and pass ihrougn to locatjails in the state-of Washington,is $2,658,279 (funding period ornpiit 1, zoz1,-tnrougn June 30, 2oz4). <br />2. SCOPEOF SERVICES' CONTRACTOR shall usethestate funds awarded lereunder <br />solely for salary, benefits, costs, and contracted services, goods, services, travel, and otheressentialcosts to support the worUprojectdefined bythe S-TATEMENT oFWoRK. <br />7. PERIOD OF OBLIGATION. <br />CONTRACT Face Sheet. <br />CONTRACT period is indicated on Line S of the <br />4. SCOPE OF WORK REI/ISIONS. CONTRACTOR shall submit to WASPC a written <br />lgquest to effect any material change to the SCOPE OF WORK identified in the STATE- <br />MENT OF WORK. Such requests shall be accompanied by a revised STATEMENT OF WORK <br />3ryt_glher supporting documents. Before any activities iupporting the revised SCOpE OFWORK are initiated,.WASPC must approve ihe same in its sote and absolute discretion,which may be withheld. <br />5. BUDGET REVISIONS. CONTRACTOR shallsubmit to WASPC a written request to <br />9ff"91any change(s) in the project budgetwhich reflect a cumulative transfer of greater than <br />len (tQ percent inthe aggregate among budget line items as indicated on the COmnnCf <br />Face Sheet. WASPC may approve or dbny the request irn its sole and absolute discretion. <br />3' SCOPE OF WORK. The CONTRACTOR shall seek to implement the activities and toachieve thegoals and objectives of the Detention & Mitigation otbOvlO-1g in Jails program, <br />as set forth in the STATEMENT OF WORK <br />6' PERFORIIIIANCE STANDARDS. The CONTRACTOR shall perform the services asdeftned in the STATEMENT OF WORK incorporated herein; in accordance with the budgetand estimated expenditure plan, as stated on tire CONTRACT Face Sheet and in accordaicewith the Detention & Mitigation of COVID-19 in Jaits Program, as well as other policies andprocedures issued by WASpC. <br />D&M COVID t9 Jails Contract -ZO2Z-OOI Page 2 of 7
