EXHIBIT "A" <br />SCOPE OF WORK <br />The Contractor shall employ one or more Community Health Workers / Promotoras de <br />Salud, who will work to decrease barriers to accessing COVID-19 vaccination for Kittitas <br />County's Latinx and Spanish-speaking households. The position requires courteous and <br />cooperative interaction with all department staff, County, State, and Federal agencies, <br />and the general public, and will be expected to provide assistance in other areas of <br />Public Health as needed. All employees work under the guidance of local/state/federal <br />regulations. <br />Essential Functions: <br />• Improve COVID-19 testing and vaccination access and uptake in local Latinx and <br />Spanish-speaking communities by: <br />• Building relationships with community leaders and organizations <br />• Identifying and implementing outreach strategies to reduce barriers to testing and <br />vaccination <br />• Increasing community COVID-19 knowledge <br />• Developing effective relationships with community members and community <br />partners <br />• Assisting with care coordination services related to vaccine access and barrier <br />reduction <br />Working in partnership with KCPHD staff <br />• Including information on other recommended vaccinations, including influenza <br />and childhood immunization series. <br />OTHER FUNCTIONS: <br />• Respond to Public Health related inquiries from the public as needed. <br />• Maintain confidentiality as described in agency, federal, and state required policies. <br />This job description is a summary of the primary functions and expectations for this <br />position. Additional duties may be assigned as required. <br />MINIMUM QUALIFICATIONS: <br />• Proficiency in verbal and written Spanish. <br />• Insight and knowledge of cultural norms in local Latinx communities. <br />• At least one (1) year of professional or volunteer health, social service, or related <br />experience. <br />• Valid Washington State driver's license. <br />Professional Services Agreement <br />Page 14 of 19 <br />
