1 <br />2 <br />EXHIBIT C <br />STATEMENT OF WORK <br />Dete ntion Se rvices <br />Pu rpose <br />To provide secure detention services to youth pending transportation to a JR residential facility that are: <br />1 ) state committed; 2) parole revoked; or 3) community facility transfers. <br />Contractor Obl igatio ns <br />a' The Contractor shall provide secure detention center services including care, custody, supervision, <br />education, and recreation to the followng JR youth while in detention: <br />(1) For youth who are committed to the State, the Contractor shall: <br />(a) Make direct contact wth the JR designated staff of commitment <br />(b) Provide and make available to JR with the following information for each youth committed to <br />JR: <br />i. Court Order <br />ii. Complete JR Sentencing Worksheet <br />iii' lnformation necessary to successfullytransition the youth, including contact infonnation <br />for the youth's parents/guardian. <br />(c) Detention stays become billable upon notrfication and receipt of the above documentation byJR (except wlren information is received on business days after4:00 pm). <br />(2) Make available the following information for each youth committed to JR: <br />(a) lnformation to the Court on the Offense <br />(b) Police Reports on the Offense <br />(c) Victim witness lnterviews (wtren completed for sex offenders) <br />(d) Previous Reports to the Court (if available) <br />(e) lncidents Reports from current Detention stays (if applicable) <br />(f) other Social File Materials (e.g., mental health reports, school information, etc.) <br />(3) For youth who are on parole revocation or, if applicable, a community facility transfer due tothreats of health or safety of others. <br />Departmentof Children, youth, and Families <br />2072C5 County Consolidated program Agreement 6_20-201g a1
