DOH CONTRACT #: CLH3t0l5 <br />Standard Federal Certifications and Assurances <br />Following are the Assurances, certifications, and special conditions lhat apply to all federally funded (in whole or in part)agreements administered by the washington State Departrnent of iealth. <br />CERTIFICATIONS <br />1.
