Exhibit A, Statements of WorkRevised as of September 15,2021AMENDMENT #23Payment Information and/orAmountReinforcing CapacityThesefunds are to each LHJ to deliver FPHS in their own iurisdiction - In coordination with the FPHS Steering Committee and Subject Matter Expert (SME)lTorkgroups, provide FPHS Communicable Disease (CD), Environmental Public Health (EPH), Assessment (Sirveitlance & Epidemiologt) and / or any or all of theother FPHS Cross-cutting Capabilities (CCC) as defined in the most cunent version oJ'the FPHS definitions.Assessment - CHA/CHIP (FPHS definitions G.3)These-funds are to each LHJ to deliver FPHS in their owniurisdiction * In coordination with the FPHS Steering Committee and Subject Maaer Expert (SME)Workgroups, conduct and complete a comprehensive community health assessment and identify health prioritiei arisingfrom that aisessment, incl)ding anaiysis ofhealth disparities and the social determinants of health as de/ined in the most current version of the FpHS definitions.t Conduct a local and/or regional comprehensive community health assessment (CHA) every three to five years in conjunction with community partners.t Develop a local and/or regional community health improvement plan (CHIP) in conjunction with cornmunity partners.These funds can be used for any CHA/CHIP activity or sentice (e.g., data analysis, focus grotups, report writing, prociss .facilitation) and may be used to contractyvith other LHJs for staff time or services.Communicable Disease (CD) (FPHS definitions C.I, 2, 3, 4, 6)These funds are to each LHJ to deliver FPHS in their owniurisdiction - In coordination with the FPHS Steering Committee and Subject Matter Expert (SME)Workgroups, provide FPHS CD services as defined in the most current version of the FPHS definitions. Thesefunds can (and actuaily are intendid to) be braidedwith temporary pandemic emergencyfitnding such that when thosefunds run out, FPHSfunds can be used to ritain staff there were h'ired with pandemic emergencyfunds if the iurisdictions desires to retain them and/or to hire additional staff if needed, and/or contract with other LHii for staff time or senic)s for deliverinl FpHSCD- As the pandemic response wains, staffunded with FPHS funds are to shift focus to providing some or all or the FPHS CD serttices. This iicludes mainlainingAccess to and use ofdata systems created during the pandemic and others under development and case investigation and contact tracingfor sexually transmitteddisease and other communicable and notifiable conditions vvithin the mandated timeframes. Emphasis should be placed on addresstng srytlit* and gonoryhea cases.I.
