(ASAM), and continued education regarding the most up-to-date, evidence-base treatment <br />available is preferred. <br />Nurse Care Manager fNCM) <br />The NCM's responsibility will be to overseeing health, wellness, and medical prosesses <br />for patients undergoing reatment at the Kitritas County Jail. The NCM will also provide <br />screening, triage, crisis intervention, medical services, case coordilation, ancl education and <br />referral services. The NCM shall utilize best practices to engage ald help motivate patients to <br />commit to treatment and to assist patients to identii/ and problem-solve individual and systemic <br />barriers to treatment. <br />Peer Support Specialist <br />The Peer Suppo* Specialist will provide support for those who are going through <br />treatment or are new to recovery utilizing their life experiences and recovery. The peer Support <br />Specialist will interface with patients individually and with the members' care team. The peer <br />Support Specialist will work patients to prepare for their transition into the community including <br />but not limited to, coordinating transportation, providing referrals to communify organizations, <br />scheduling treatment follow-up, and applying for health insurance. <br />PIan for Collecting the Data Required for this Solicitation's Performance Measures <br />Kittitas CounS Sheriff s Office believes strong program evaluation can help identifli the <br />best investments as well as determine how to establish and sustain them as evid.ence-based best <br />practice.
