Kittitas Counry Sheriffs Office has developed 42 CFR part2 compliant consent forms that encompass all requirements set forth by the Legal Information Institute and the L,egal Action Center. All staff are fully kained in the utilization of consant forms prior to start of work. Ongoing training is provided annually, or sooner, as rules change. The elements of each consent are fully understood. We utilize rwo types of consent forms: Consent to Treat and Consent to Release Information. Each consent fonn has the Prohibition on Redisclosure statement. All pafticipants will be fuily informed of their right to sign, revoke, and/or limit the consent form. These forms will be maintained within participant file and datelevent of expiration will be monitored. The initiating site will maintain the original consent fonn and a copy will be provided to the referent. No participant information will be disclosed without a valid consent form being completed, unless otherwise allowed by law {medical emergency, mandatory duty to report, etc,). Under 42 CFR Part2, Subparts 34 and 35 disclosures may also be made to other correctional facilities and to judicial agencies when participation in a program is a condition of their release or parole. AII elements of these subparts will be followed and documented within the participant file. Capabilities and Competencies Our initial concems focus primarily on being able to appropriately staff the jail with qualified and culturally informed personnel. A MAT partner organization will need to have developed a list of appropriate candidates for the positions needed with the ability to have a full sraff no later tha;n12/0112020 and will be able to provide services by 12/31/2020.
